Maester Ballabar follows Lord [[Paxter Redwyne]] to [[King's Landing]] after the [[Battle of the Blackwater]]. At the request of Queen Regent [[Cersei Lannister]], Ballabar is assigned the task of caring for [[Tyrion Lannister]], who was seriously injured in the battle. Ballabar keeps Tyrion unconscious with [[milk of the poppy]].{{Ref|ACOK|67}}

After Tyrion wakes, he grabs Ballabar's chain and strangles him with it, releasing the links only when the maester's face begins to turn purple. Tyrion tells Ballabar he wants no more poppy, and against the maester's advice, makes Ballabar remove his bandages and show him his wounded face. Tyrion wishes to be taken from [[Maegor's Holdfast]] back to his rooms, but Ballabar has to inform him that his father, Lord [[Tywin Lannister]], has taken up the apartments in the [[Tower of the Hand]] and the position of [[Hand of the King]] after returning to King's Landing and winning the Battle of the Blackwater.{{Ref|ACOK|67}}

Because Tyrion suspects Ballabar received orders from Cersei to kill him, he tells his [[squire]], [[Podrick Payne]], to send Ballabar away and instead get [[dreamwine]] from [[Frenken]], the [[Stokeworth]] maester.{{Ref|ACOK|67}}

During the [[Purple Wedding|royal wedding]] of King [[Joffrey Baratheon|Joffrey I Baratheon]] to [[Margaery Tyrell]], Ballabar treats a [[Dornishmen|Dornishman]] after he is stabbed by a [[House Rowan|Rowan]] [[knight]].{{Ref|ASOS|60}}

After the death of Joffrey at his wedding, Ballabar and Frenken perform the autopsy that refutes the theory of the king's suffocation on the wedding pie. During the trial of Tyrion for the death of Joffrey, Ballabar is called as a witness for the prosecution to present the findings of the autopsy, and he declares that [[poison]] was used.{{Ref|ASOS|66}}
